This is the online application process for all volunteers of Great Shape!’s 1000 Smiles Dental Project . Before you apply, please read all the information about the 1000 Smiles Project on our website.

There are 2 Parts to the Application process:
  1. Part 1: Create Your Account - You will create a user account with Great Shape. Your information will be confidential and you may stop at any time during the process and return to it later. Write down your user name and password.

  2. Part 2: Application – You will complete steps 1-4 with your basic information. If your application is approved by email, you will then complete part 3 of the registration process.
