This is the online registration process for all volunteers of Great Shape!’s 1000 Smiles Dental Project.    Before you apply, please make sure you have read all the information about the 1000 Smiles Project on our website.

There are 3 Parts to the registration process:
  1. Part 1: Create Account - You will create a user account with Great Shape so your information will be confidential and so you may stop at any time during the process and return to it later.

  2. Part 2: Application – You will complete steps 1-4 with your basic information. After your application is approved by email, you will be able to complete part 3 of the registration process.

  3. Part 3: Registration: Documents & Fees – Here you will complete steps 5-7 and complete the necessary documents required to volunteer, send us your volunteer fees and send us your travel itinerary.
